A. AGREEMENT AND RELEASE

In consideration of  "GD SPORTS LIMITED" /”GYMDanny”/ “School Gymnastics Program” furnishing training and instruction for “student”, we, the parents of “student”, hereby agree and do hereby release, remise and forever discharge "GD SPORTS LIMITED"/ ”GYMDanny” / “School Gymnastics Program”, its shareholders, employees, agents, officers, directors, attorneys, successors or assigns of and from all, and all manners of, actions and causes of action, suits, debits, dues, accounts, bonds, covenants, contracts, agreements, promises, judgments, claims and demands whatsoever in law or in equity, especially arising out of or in connection with the furnishing of gymnastic instruction, services, training and recreational services provided by "GD SPORTS LIMITED"/ ”GYMDanny” / “School Gymnastics Program “regarding “student”, which against the said  " GD SPORTS LIMITED"/ ”GYMDanny” / “School Gymnastics Program”, its shareholders, employees, agents, officers, directors, attorneys, successors or assigns, the parents of “student” or “student” ever had, now have, or which their respective heirs, executors, administrators, successors or assigns, or any of them, hereinafter can, shall or may have, for or by reason of any cause, matter or thing whatsoever, and since "GD SPORTS LIMITED"/ ”GYMDanny”/”School Gymnastics Program” assumes no responsibility for any injuries or accidents which arise while “student” is exercising or performing gymnastics or both at the School venues, other venues in the territory and/or overseas venues, it is extremely important that “student” be in good health and physical condition since previous illnesses or injuries could be complicated by such physical exercise.  The “students” are further advised to consult their personal physician to determine if “students” should engage in such exercise or gymnastics or both at the Gymnastics Program, prior to commencement of such exercise or gymnastics training program.

I/we authorize "GD SPORTS LIMITED"/ ”GYMDanny” / “School Gymnastics Program”and/or representatives to administer basic first aid at the training areas and/or competition site(s) in case of injuries or when medical attention is deemed to be necessary.  In the event of an emergency, when contact cannot be made with the parents, "GD SPORTS Ltd"/ ”GYMDanny” / “School Gymnastics Program”and/or its representative will take the “student” to the Hong Kong Adventist Hospital or the nearest government recognized hospital.  I/we do not hold "GD SPORTS LIMITED'/ ”GYMDanny” / “School Gymnastics Program”, its shareholders, employees, agents, officers, directors, attorneys, successors or assigns, and other individual or organization connected with "GD SPORTS LIMITED"/ ”GYMDanny” / “School Gymnastics Program” responsible for authorizing treatment or for any costs/expenses resulting from such treatment.  I/we also authorize the Hong Kong Adventist Hospital or government recognized hospital to perform any emergency care deemed necessary for the treatment of my child.  I further understand that my child may be transported by  "GD SPORTS LIMITED"/ ”GYMDanny” / “School Gymnastics Program”to and from meets/performances and realize that this Release covers any accident or injury while in transit.
